Abstract

Latar Belakang: Stunting merupakan prioritas utama kesehatan global. Di Indonesia, stunting masih menjadi masalah yang kini semakin dikaitkan dengan disbiosis mikrobiota usus. Penelitian ini bertujuan untuk melihat profil mikrobiota usus anak-anak stunting di Banyumas, Indonesia. Metode: Studi analitik observasional dengan rancangan cross-sectional dilakukan terhadap 30 anak (usia 2-5 tahun) di Desa Watuagung. Status gizi ditentukan melalui antropometri, sedangkan pola makan dipantau melalui kuesioner. Profil mikrobiota usus diidentifikasi dari sampel feses menggunakan pengurutan gen 16S rRNA V1-V9 berbasis Oxford Nanopore Technologies. Hasil: Anak stunting menunjukkan keanekaragaman (alpha diversity) dan kekayaan spesies (Richness) mikrobiota yang lebih rendah dibandingkan kelompok gizi normal, menandakan ekosistem usus yang lebih sederhana. Meskipun filum Bacillota mendominasi kedua kelompok (>96%), kelompok stunting memiliki proporsi ordo terkait Clostridia seperti Lachnospirales dan Eubacteriales yang lebih tinggi. Sebaliknya, bakteri protektif seperti Lactobacillus dan Agathobacter lebih melimpah pada kelompok gizi normal. Analisis fungsional PICRUST2 berbasis data KEGG menunjukkan peningkatan aktivitas metabolik mikroba pada jalur karbohidrat, asam amino, dan energi pada anak stunting. Hal ini konsisten dengan data diet yang menunjukkan asupan makronutrien dan mikronutrien yang signifikan lebih rendah pada kelompok stunting. Kesimpulan: Terjadi disbiosis fermentatif pada anak stunting di Banyumas, ditandai dengan aktivitas metabolik tinggi namun tidak efektif. Kondisi ini mengganggu penyerapan nutrisi dan pertumbuhan, sehingga diperlukan strategi intervensi yang mengintegrasikan perbaikan nutrisi sekaligus pemulihan mikrobiota usus. Background: Stunting is a global health priority. In Indonesia, it remains a significant issue increasingly linked to gut microbiota dysbiosis. This study aims to analyze the gut microbiota profile of stunted children in Banyumas, Indonesia. Methods: This observational analytic study used a cross-sectional design involving 30 children (aged 2–5 years) in Watuagung Village. Nutritional status was determined via anthropometry, while dietary patterns were monitored through questionnaires. Gut microbiota profiles were identified from fecal samples using 16S rRNA V1-V9 gene sequencing based on Oxford Nanopore Technologies. Results: Stunted children exhibited lower alpha diversity and species richness compared to the normal nutrition group, indicating a simpler gut ecosystem. Although the phylum Bacillota dominated both groups (>96%), the stunted group showed a higher proportion of Clostridia-related orders, such as Lachnospirales and Eubacteriales. Conversely, protective bacteria like Lactobacillus and Agathobacter were more abundant in children with normal nutrition. Functional analysis using PICRUST2 and KEGG pathways indicated increased microbial metabolic activity in stunted children, particularly in carbohydrate, amino acid, and energy metabolism pathways. These findings are consistent with dietary data showing significantly lower intake of macronutrients and micronutrients in the stunted group. Conclusion: This study identifies a distinct fermentative dysbiosis in stunted children in Banyumas, characterized by high but ineffective metabolic activity. This condition may impair nutrient absorption and growth, highlighting the need for targeted nutritional interventions that integrate dietary improvement with microbiota restoration.
